    Supported self-management is when health professionals, teams and services work in ways to ensure that people with long term conditions have the knowledge, skills and confidence to manage their health, as part of their everyday life. This leads to better health outcomes, and a better experience for people with long-term conditions.

    Supporting people to be more actively involved in managing their health is not always easy. We have developed a Self-Assessment, which helps health teams to understand how well they are currently doing. Questionnaires are completed by people who use services, and people who plan, manage and deliver care. The answers are compared, and are used to help health teams to decide how to improve their service. Support is then given to make these improvements. The Self-Assessment can be repeated at the end, to understand what has changed. This process of self-assessment, quality improvement and repeat self-assessment is referred to locally as WASP (Wessex-Activation and Self-Management Project).

    Across the Wessex region, a number of teams have opted to follow the WASP process. These teams are completing the self-assessment, and are receiving quality improvement support to make improvements relating to personalised care.

    This project will evaluate the WASP programme of work. We will do this using different methods to better understand if it works, how it works and how it can be improved. This will include comparing the questionnaire responses before and after the programme, interviewing members of the team before and after the programme and observing the planned programme sessions. This will help us understand how the coaching and support works, and how it can be improved.
