    The Selby Ageing Well and Frailty Team were commissioned in October 2022, with a key remit of being proactive in the identification and management of frailty in older adults in our local community. There is a large evidence base about how frailty can be prevented, delayed or reversed by an individual making positive lifestyle changes to their diet, exercise and social interactions and the team seek to encourage these changes in individuals who have been referred to the service. However, there is a need to target those individuals who might be at risk of frailty or living with frailty but are not currently in receipt of any services.
    The team has worked in partnership with York and Scarborough Teaching Hospitals NHS Foundation Trust and Selby Town Primary Care Network (PCN) colleagues, along with North Yorkshire Council and local Voluntary sector organisations to form an Ageing Well Partnership. This partnership has produced a joint vision on how to promote an Ageing Well agenda, with Ageing Well Health Fairs at the centre of this approach. We have run four well attended Ageing Well health fairs but would like to know if these events are valued by the local population of older adults, and if there are any barriers in attending the events. We would also like to explore if the Ageing Well Health Fair is a good concept and discuss if there are any actions we can take to make the fairs easier to attend, engage and interact with.
    The local GP practices identify certain cohorts of patients that are then sent invite letters informing them about the event. The partnership also advertise the event through leaflet flyers, posters, social media adverts and word of mouth.
