Research summary
Young people with type 1 diabetes (T1D) are at risk of developing damage to the nerves in their feet and hands, a condition known as diabetic peripheral neuropathy (DPN). This can cause pain (Painful-DPN), numbness, and an increased risk of serious foot problems such as ulcers and amputations later in life. Traditional tests used in routine care often only detect nerve damage once it is advanced and permanent.
Our research team has previously screened young people (aged between 18 and 25) with T1D in Sheffield using more sensitive, objective tests that can identify early nerve changes. At that time, we found that while very few had detectable nerve damage with standard tests, around 8% showed early signs with advanced testing, and 3% already had painful nerve damage.
This follow-up study, called STRIDE-T1DM, will invite the same group of young people back 3–7 years after their first screening. We will assess whether they have developed DPN or Painful-DPN, and what risk factors (such as blood sugar control or duration of diabetes) are linked with these complications.
Participants will attend one study visit lasting 30–60 minutes at Sheffield Teaching Hospitals. They will have a foot check, short questionnaires, and two quick, non-invasive tests: one to measure sweat gland function (SUDOSCAN) and another to measure nerve signals in the leg (DPN-Check). We will also access their medical records to extract routine eye screening and blood and urine test results.
By understanding how and when DPN develops in young people with T1D, and what factors increase risk, this research aims to improve early detection and support the development of strategies to prevent progression. The results will be shared with participants, presented at scientific meetings, and published in journals.
