Research summary
The role of HLA specific antibodies in renal transplantation and subsequent graft loss has been extensively studied. The frequency and clinical importance of non-HLA antibodies in renal transplantation is not completely understood. Non-HLA antibodies may be directed against a variety of non-HLA antigens which include various minor histocompatibility antigens, vascular receptors, adhesion molecules, and intermediate filaments. Several studies have implicated these antibodies in graft rejection as some patients with no evidence of donor directed HLA specific antibodies have experienced humoral rejection and or Transplant microvasculopathy. These antibodies have received sporadic attention with respect to the overall incidence and any impact on the overall health of transplant patients. It is well documented that patients with renal disease have a high risk of cardiovascular disease which is not completely ablated following transplantation. The role of non-HLA antibodies and their contribution to ongoing damage to the systemic vasculature in renal patients has not been studied. We suggest that Non HLA antibodies may be part of an inflammatory response which may predisposes to vascular injury, we hypothesise that this has a detrimental effect on cardiovascular function and contributes ultimately to the high rate of cardiovascular disease in renal transplant recipients.
