Research summary
An increasingly elderly population with complex health and social care needs is putting the health system under severe pressure. There is a growing need to find ways to better manage this population within primary care, to optimise their care and reduce unplanned admissions to hospital. The Northumberland High Risk Patient Programme (NHRPP) was introduced in 2012 as a way of achieving this. It aims to identify patients who are at risk of unplanned admission and offer them a package of targeted medical assessment and proactive personalised care planning to optimise their care in the community and address their future care priorities. This study aims to explore the experiences of patients on this programme, as well as those of their informal carers, to understand what impact it has had on them.
A separate work package will evaluate the views of the multi-disciplinary team on the NHRPP.
