Research summary
This study explores whether adopting a Population Health Management (PHM) approach can improve the experience of end-of-life (EOL) care (Using data that has already been collected).
Within North East Essex (NEE), a PHM approach has been used since 2018. The document called “10 Outcomes that Matter”, was developed and created through collaborative discussions with the Patient Participation Group (PPG), staff, and organisations that care for and support people at EoL. These outcomes are captured in data from advance care plan discussions and recorded on the My Care Choices Register (MCCR) electronic system. The PHM approach aims to help identify inequalities, influence service delivery and approaches to reduce inequalities and improve people's experiences.
The research will review projects from health and social organisations within NEE using the data to address inequalities. The reports they have produced and presented will be included and used to evaluate their effectiveness. Staff members involved will be asked to complete a short anonymous online survey to show their understanding of PHM and determine whether they feel it has improved patient access and experience. The aim is to review how effective the intervention has been in addressing inequality.
Patients, carers/loved ones will be recruited for interviews to share their experience of EoL care, what’s important to them, what went well or not, and any recommendations for future service changes. The interviews will take place face-to-face at a location of the participant's choice, which may be their own home, and last approximately one hour. No personally identifiable information will be used in the final write-up. The interviews will be compared to the PHM outcomes and data collected to ensure that what is being collected reflects what is important to the people receiving or being supported.REC name
