    All complex rehabilitation units in the UK have to record and submit a set of data that includes the Functional Independence Measure and Functional Assessment Measure (UK FIM + FAM)(Turner-Stokes et al 1999). There are measures of function and disability in brain injury covering the following four areas: activities of daily living, sphincter management, mobility and executive function. In patients with especially complex needs, this measure is not sensitive enough to show change in function, especially from a physical perspective. For example, a patient may progress from being passively hoisted into a chair using a full sling, to being able to contribute to the effort of using a stand hoist, but they will still score the same on the “bed, chair, wheelchair transfer” section of the UK FIM + FAM, despite clinically significant progress in functional ability.
    Scores from these two established measures will be compared with scores from a new instrument, the Chelsea Critical Care Physical Assessment (CPAx) (an observational measure of function and respiratory status in critically ill patients), to see whether the scores for the physical elements correlate and to compare their sensitivity to change in this patient group. The aim is to assess the feasibility of using the CPAx tool in its current form with rehabilitation patients.

    The proposed study will take place in the Walton Centre, an acute complex rehabilitation setting in the North West of England. Physiotherapists working on the unit will collect the study measures. Physiotherapists will complete UK FIM + FAM for each patient on admission and discharge according to standard practice. The CPAx will also be scored by two separate therapists for each patient on admission, to establish reliability of the measurement process, and by the treating therapist prior to discharge from the unit.
