Data collection arrangements
We will include in the database routinely collected information on all patients attending Bristol hospitals for treatment following a self-harm episode. These will include characteristics of the patients such as age and sex, information about the episode of self−harm such as the method used and date and time of act, the support the patient received during their attendance in the hospital such as receipt of psychiatric treatment, and history of mental health problems.\n\nData will be collected by a designated member of the research team and supervised by the data custodian and the Chief Investigator. Data would be extracted from hospital records onto data collection sheets to facilitate data entry. Data would be entered onto a computer linked to the hospital trust secure server and would not be stored on the computer hard drive. Only the database custodian and the person maintaining the database would have access to the database and to the locked filing cabinet in which the paper data extraction sheets would be stored. Only they and members of the clinical team will have access to patient identifiable data.\n
Research programme
The Bristol Self-harm Surveillance Register has been recording detailed information on patients presenting to hospital for self-harm since 2010. The database is maintained in the Emergency Departments of the Bristol Royal Infirmary, part of University Hospitals Bristol NHS Foundation Trust (UHBT) since 2010, and Southmead Hospital (initially Fenchay Hospital, prior to the transfer of acute services from Frenchay to Southmead), North Bristol NHS Trust (NBT) since 2013. \n\nInformation recorded on the register enables an assessment of i) the incidence of hospital-presenting self-harm in Bristol and its geographical distribution, ii) trends in the incidence of self-harm and its management; iii) the impact of changes in service delivery on patient management and outcomes and iv) risk factors for repeat self-harm and suicide; v) the medicines taken in overdose. This information also contributes to local prevention efforts and to the STITCH (Services and Trusts Integrating to Transform Care in Self-harm) Health Integration Team (HIT). These data also provide insight into the management of patients for clinicians and managers while also allowing the evaluation of services against NICE guidelines for treatment of self-harm patients.\n
