Research summary
Type 1 diabetes (T1D) is one of the most common chronic diseases in childhood. T1D requires complex treatment plans and lifelong self-management. With approaching adulthood, adolescents experience the transition from pediatric to adult health service. There is increasing evidence that an unsuccessful transition may result in discontinuity in treatment, a decrease in doctors’ visits, an increase in changing physicians, and an increase in poor health outcomes.
Formal guidelines have been proposed by multiple professional organizations focusing on planning the transition from pediatric to adult care which include the identification of patient-specific health care needs, education, and disease management.
This study will comprise of two phases. First adolescents with T1D aged 16-19 years, under the care of the Diabetic Paediatric Care Team at Nottingham University Hospitals NHS Trust, will be asked about readiness for transition to adult health care settings through a self-administered survey. Secondly, a selection of respondents to the survey will be interviewed to ask them about their experience, expectations and preferences for transition to adult health care.
This study aims to explore the experience of the adolescents with T1D regarding their transition to adult health care settings, to understand in more depth the factors that may support their transition from pediatric to adult health care setting.REC name
