    Achieving preferred place of care and death are important measures of success for palliative care services. However, in the literature there is little data on the end of life preferences of older adults living in care homes and so the views of this population are not being represented. The aim of the study is to investigate the end-of-life care preferences of elderly people living in residential care homes. Resaons why people make these choices will also be considered. We will include people aged greater than sixty five years who are permanent residents of a residential care home and have been resident there for more than 2 weeks. People will be excluded if they have a problem such as dementia which would mean they are unable to understand the study and make a decision whether or not to take part. The researchers are based in hospitals in Guilford-Surrey, Northwood- Middlesex and Lambeth- London. They will be approaching people living in the residential care homes in the surrounding areas.
    The study is a prospective observational study. Participants giving informed consent to enter the study will be asked to complete a short questionnaire. The questionnaire asks about their wishes for future care if they became unwell and preferences for where they would wish to be cared for and die. The consent process, and completion of the questionnaire should take approximately 20-30 min.
