    Rationale
    Heart failure is associated with unacceptably high morbidity and mortality rates and poor quality of life, 30 – 47% of patients with heart failure also suffer from diabetes which is one of the frequent co-morbid conditions. The co-existence of two long term conditions poses great challenges for self-management with patients needing to monitor multiple symptoms and physiological parameters, as well as compliance with complex treatment regimens as part of self-care. However little is known whether they receive adequate support from healthcare providers for their self-management of heart failure and diabetes, what are the difficulties of living with these two conditions, and what patients need to integrate self-management.

    Aims of the study
    The aims of this project are to understand current self-management in heart failure patients with diabetes, to identify support received from healthcare providers and support needed to promote self-management, to identify any potential gaps in current nursing practice including both primary and secondary care services.

    Target population
    Adult heart failure patients with documented diagnosis of diabetes.

    Health setting:
    Heart Centres, diabetes treatment centres at Central Manchester NHS Foundation Trust and other trusts in North Ireland and Cambridge.

    Study design
    Two stage descriptive study with mixed methods
    Stage 1: consented patients with heart failure and diabetes will be asked to complete a questionnaire on their self care behaviors, self efficacy, mood and medication adherence related to heart failure and diabetes.
    Stage 2: purposively selected participants (approximately 20-30) who have completed the questionnaires with willingness to attend an interview will be invited to an in depth Interview. The interview will last 30-60 minutes to explore patients’ perspectives of self management of heart failure and diabetes, difficulties in managing the two conditions, and their need for support from healthcare providers.
