Research summary
We know that older people can experience difficulties with their health, well-being and day-to-day activities. We also know they don’t always receive the support they need. The Support Needs Approach for Patients (SNAP) helps people consider where they need more support and discuss these needs with a healthcare professional. SNAP consists of a short set of questions for patients to consider and a conversation, based on their responses, between the patient and healthcare professional. SNAP has been used successfully with (mainly older) people with Chronic Obstructive Pulmonary Disease. SNAP enabled them to identify their need for support across a range of areas and led to needs being met (including support with loneliness and getting out-and-about). We now want to know whether SNAP can be used more generally with older people. This study aims to explore SNAP with older people living in the community who are being visited by district nurses. In order to do this, we plan to interview about 20 older people who have used SNAP about their experiences and any benefits they gained. We also plan to talk to the nurses who delivered SNAP to see whether they feel it is useful and easy to use in their particular role.
