Research summary
Delirium is a frequent complication of critical care, causing increased length of stay, higher morbidity, and increased mortality. This is thought to be linked with poor sleep in critically ill patients, which is impacted by high levels of ambient sound and light in addition to patients being frequently disturbed by procedures and provision of care. We locally trialled a low cost, non-pharmacological intervention comprising a multi-component, multi-disciplinary bundle of measures to limit iatrogenic sleep disturbance, which improved patients’ subjective experience of sleep, and halved the incidence of delirium. However the adoption of these measures to sustain improvements for patients outside of research conditions is the next challenge. We now intend to introduce across the regional critical care network a toolkit to promote sustained implementation of the bundle, using SoundEar devices to act as “hands-off” investigators, raising awareness of ambient noise at night and reminding staff that the bundle is in place. To achieve this we are going to pilot the bundle in Royal Preston Hospital, assessing its utility and acceptability prior to network-wide release.
