Research summary
Discharge from hospital for frail older people works best when they are fully involved in planning for their discharge with health professionals. This is called shared decision-making. However, shared decision-making about discharge from hospital is a challenge for patients and health professionals and we do not know the best way to do this.
We will work with patients, their relatives, and health professionals to develop and test an intervention to help frail older people be fully involved in planning for their discharge from hospital.
