  • Research summary

Why: This project will look at what factors can be used to most accurately diagnose stroke-associated pneumonia (SAP).
What: Pneumonia is the most common and serious complication of stroke and is linked to longer hospital stay, poor recovery and death. It most often occurs within seven days of stroke onset and those who are most at risk are patients with swallowing problems (dysphagia), low conscious level, poorest mobility and low immune function. Accurate diagnosis of stroke is important to ensure treatment is appropriate and timely. However, at Salford Royal NHS Foundation Trust there are currently no formal approved guidance for diagnosis of SAP, so the decision to commence antibiotics is based on clinician’s own judgement and Trust policy.
Who: We will not recruit participants to this study. The Sentinel Stroke National Audit Programme (SSNAP) is a national audit of stroke care from all stroke units in England and Wales. It collects general data relating to SAP (e.g. diagnosis, treatment and outcome) but does not detailed information that could inform a diagnosis. Although SSNAP data is anonymised (not possible to identify individuals), each site has a link to its own data which can be linked to individual patient data by members of the direct care team.
Where/How: The research team at Salford will identity cases of SAP from their SSNAP data and extract additional information from the patient’s electronic patient record (EPR) e.g. breathing rate and temperature, antibiotic treatment and length of stay. The project will take approximately 12 months and the results could potentially inform other antibiotic treatment trials and/or SAP related quality improvement studies.
