Research summary
This study is part of a programme of work to improve how we respond when a patient deteriorates after emergency surgery on the abdomen. We have studied existing systems for responding and developed ideas for improvement by working with frontline doctors and nurses. In this study we will try out the new systems in three hospitals. If they seem to work well we will go ahead with a major national trial in 24 hospitals next year. We will change 4 things about the response process: First, we will introduce a way for patients to call an emergency themselves if one of the other patients with them becomes ill; Second, we will change parts of the communications and actions pathway to make it clearer what to do and when to do it and to minimise errors and delays; Third, we will give staff training to strengthen teamwork, based on learning from elite sports and military coaches; Finally, we will improve co-operation between departments, using joint simulation exercises, positive feedback and recognition, and a standard technical way of describing ill patients to improve understanding.
We don’t think we will have enough cases in a year to prove that the new systems are better for patients, so we will study whether we can prove they result in a reduction in the number of patients needing further surgery or intensive care, the time it takes teams to respond and the quality of the response.
