Research summary
Research objective is to assess if 30-day emergency readmission in dialysis patients can be reduced by following interventions\n•\tImproving communication of treatment changes between discharging unit and satellite dialysis units.\n•\tEarly blood tests post-discharge (within 1 week) and review.\n•\tEarly (within 2 weeks) review by clinician post discharge.\nHealth status assessment using EQ 5D scores will be conducted in patients to assess the impact of admission and readmission on their quality of life.
