Research summary
For invasive primary cutaneous melanoma, a secondary wider excision around the original biopsy scar is advocated to reduce risk of local recurrence and improve patient outcomes. Surprisingly, the detail of the wide excision is still controversial and the management of patients with intermediate and high-risk primaries (AJCC pT2-pT4) is particularly ambiguous. International guidance on surgical margins for these groups of patients varies from 1 cm to 3cm (translating into large defects from 2 cm to 6 cm in maximum diameter) depending on Breslow thickness. The aim of this study isto determine the reconstructive and resource burden of wider excision margins.
