    The TOHETI project (Transforming Outcomes and Health Economics Through Imaging) has been funded by the Guy’s and St Thomas’ Charitable Foundation to transform existing clinical pathways through the use of relatively new imaging technologies. Centred round clinical services at St Thomas’ Hospital the project aims to both improve patient experience and achieve significant cost savings for the NHS. While the main innovation of the TOHETI project is the introduction of new imaging technologies to change existing patterns of care it has been recognised that changing clinical services can be challenging and in consequence the project is embedded in a model of organisational change that will be led by a number of champions/implementers. At the end of the project there should therefore be evidence as to whether or not the pathways have been improved and whether or not patient experience and cost targets have been achieved.
    The TOHETI project involves reconfiguring eight clinical pathways and it is likely that some of these changes will be more successful than others given the technological, organisational and cultural context in which they will be introduced. The purpose of this implementation evaluation is to use qualitative research methods to try and establish when, why, how and for whom the changes work best and the reasons why they may be less successful; an understanding of the factors that engender success should prove useful for future change initiatives in the NHS.
