Research summary
Developing a Question Prompt Lists for people with advanced liver disease (cirrhosis). Liver clinicians are uncomfortable about discussing prognosis with patients. Other methods are needed to support professionals with communication in this area. Question Prompt Lists (QPLs), one page lists of “recommended questions” given to patients before their outpatient consultation, may be one approach. Previous studies indicate they may improve in communication during the out-patient appointments, but most QPLs have been designed for patients with cancer. This study will develop and pilot a Question Prompt List specifically designed for people with cirrhosis and their family members. To develop items for this tool, we will recruit the following: people with cirrhosis and their close family members attending the Royal Free London liver clinics, and liver doctors working at the Royal Free and their liver doctors. 10 people with cirrhosis, 10 family members and 10 doctors will be take part in either one interview or focus group, lasting no more than 45 minutes, to identify items for the liver QPL. An expert panel of researchers, clinicians and patients will develop the first version of the liver QPL. This version will be tested on another 10 people with cirrhosis, 10 family members and 10 doctors from the Royal Free Liver services. They will be given the QPL and asked to read it. Each participant will be asked to score each item for relevance, suitability and readability, and to add any additional questions. Our expert panel will reconvene to finalise the QPL using the comments obtained. We will carry out pilot work in outpatient clinics with 10 doctors using the QPL with patients over 3 months. During this time, we will invite 15 patients to comment further on the wording and content of the QPL and any issues regarding its delivery in clinic.
