    Title: Is it possible to run a large, cluster randomised control trial (cRCT) comparing Personalised Care Planning (PCP) in older people?

    Frailty is a condition of increased vulnerability to major changes in health as a result of seemingly small problems, such as an infection or new medication. It is common in older age, affecting around 10% of people aged over 65, and develops because as we get older our bodies change and can lose their inbuilt reserves, for example we lose muscle strength. People with frailty are at increased risk of falls, disability, loneliness, hospitalisation and care home admission. These problems can reduce quality of life and are costly for the NHS and social care.

    PCP is designed to improve self-management skills and improve wellbeing of older people with the support of their community. It also aims to improve coordination of GP, voluntary sector and social care services and increase the social networks of older people with frailty.

    This study aims to test if it is possible to run a large trial to see if PCP can improve the quality of life of older people with frailty and provide value for money.

    We aim to recruit 8 GP practices across Leeds and Bradford. The GP practices will either introduce PCP or continue care as usual. Participants will be identified from healthcare records by their age (over 65) and level of frailty and be sent information about the study. If they are willing to take part and for their data to be collected, consent will be sought and baseline assessments completed in their own home. If their GP is in the PCP arm, they will be offered a 12 week programme with a personal independence team. Follow up assessments will be completed at 6 and 12 months.
