    Title: Does personalised care planning (PCP) improve the quality of life of older people with frailty and is it cost-effective?\n\nFrailty is a condition of increased vulnerability to major changes in health as a result of seemingly small problems, such as an infection or new medication. It is common in older age, affecting around 10% of people aged over 65, and develops because as we get older our bodies change and can lose their inbuilt reserves. People with frailty are at increased risk of falls, disability, loneliness, hospitalisation and care home admission. These problems can reduce quality of life and are costly for the NHS and social care.\n\nOur intervention is designed to improve self-management skills and improve wellbeing of older people with the support of their community. It also aims to improve coordination of GP, voluntary sector and social care services and increase the social networks of older people with frailty.\n\nA feasibility study found it was possible to run a large trial to test if this intervention can improve the quality of life of older people with frailty and provide value for money.\n\nWe aim to recruit practices across Yorkshire & Humber and the North West. Participants will be identified from healthcare records by their age (over 65) and level of frailty and be sent information about the study. If they are willing to take part, consent will be sought and baseline assessments completed in their own home. If they are in the intervention arm, they will be offered a 12 week programme delivered by Age UK. People in the control arm will continue with their care as usual. Follow up questionnaires will be completed at 6 and 12 months to see if there is a difference between the two groups.
