    The number of older people is increasing leading to a rise in the number of people living with multiple illnesses at the same time. A new service has been developed in Hull to redesign care of older frail people, through a new Integrated Care Centre (ICC) and within the care home has shown to have a social benefit and is cost-effective. Experiences of the services have been mainly positive however there were concerns around follow-up and subsequent care as well as advance care planning (ACP). We will explore in-depth the issues (facilitators and barriers) around follow-up and subsequent care and also concerns about ACP. We will also further explore and understand the experiences of assessment by the ICC frailty team after care home admission (part of the ICC service).

    We will achieve this by dividing the study into 2 work packages. The first work package will involve interviews with patients (who have been assessed at the ICC) and related health professionals from GP practices/community and the ICC. The second work package will involve a short survey and a follow-up interview of patients to gain insight into the care home assessment process. The findings are expected to help improve the health and wellbeing of older people at risk of frailty and help inform a future redesign of the ICC service.
