    Why?
    The benefits of recording a prehospital electrocardiogram (PHECG) in patients with stroke are unknown, thus the recommendations made in the national ambulance clinical guidelines to perform this test (Joint Royal Colleges Ambulance Liaison Committee - JRCALC) are based on expert consensus rather than robust evidence.

    What is being studied?
    Phase one is studying the use of prehospital 12-lead electrocardiograms in acute stroke patients and their association with functional outcome – the ability for people to perform specific activities of daily living following a stroke.
    Phase two is investigating the perceived value in recording a prehospital 12-lead ECG in stroke patients by the stakeholders (ambulance and hospital staff) involved in their care.

    Who?
    Patients who have suffered an acute ischaemic stroke or an intracerebral haemorrhage and have been admitted to hospital by the ambulance service. The study is excluding patients who have had a transient ischaemic attack or subarachnoid haemorrhage.

    Where?
    Four sites will be used in this study. Three participating hospital sites and one ambulance service that takes patients to all of the hospitals involved.

    How?
    For phase one, data will be collected retrospectively; therefore there is no direct patient involvement. Anonymous and de-identified data from the hospitals will be linked to ambulance service data and analysed.
    For phase two, participants will be interviewed for around one hour.
