We are researching patients experience of healthcare in two orthopedic departments within two NHS Trusts that have been categorised on their performance in relation to emergency readmission's, and patient reported outcomes measures where one is categorised as excellent and the other good. Our initial study (Reference: 15/LO/0811) explored the characteristics of these departments within elective hip and knee surgery to identify the areas of positive deviance in healthcare delivery. Positive Deviance refers to an individual(s) or organisation that demonstrate a way of doing that differs or departs from the norm, but that demonstrates positive outcomes. We did this using an ethnographic approach, observing and interviewing healthcare professions on their experience of delivering healthcare and their approach to finding solutions to any issues. We want to build on these findings by understanding the patients experience of this care to identify how their perspective relates to that of professions; in particular to understand if positive deviance is recognized by patients.We will use qualitative, in-depth interview technique with patients in their home, 4 weeks following discharge from having hip or knee surgery. Patients (identified during the ethnographic study as representing a diverse representative sample i.e age, ethnicity, gender)will be approached by one of the researchers and invited to participate in the study at a later date.
Patients will be invited to participate in the individual interviews, which will be conducted in patients homes and last 45-60 minutes. Interviews will be audio taped, and transcribed. We will use a method of analysis called 'framework analysis'. We will produce findings that reflect the independent NHS Trusts, and comparative findings with that of findings from our earlier study that draws on 'comparative analysis'to identify if positive deviance influences patients experience.REC name
