Research summary
University of Plymouth employed Researchers-in-Residence (RiRs), embedded in the Integrated Care System for Devon (ICSD), will undertake an evaluation of its two-year Population Health Management (PHM) programme.
The PHM programme seeks to provide data intelligence and support to primary care and other health service providers using the One Devon Dataset (ODD) to plan health and care services collaboratively. The ODD integrates primary and secondary health and social care data across Devon, including most GP practices.
As ICS Devon rolls out its PHM programme over the next two years this research partnership between the University of Plymouth and ICSD will investigate how best to bring together data analysts, managers, and clinicians in Primary Care Networks (PCNs)/Local Care Partnerships (LCPs). It will address the following research questions:
RQ1: What factors facilitate or hinder roll-out of PHM at PCN, LCP and system level across Devon ?
RQ2: What are qualitative and quantitative outcomes of patients involved in PHM innovations?
RQ3: What are the real or potential economic benefits to the system?
To do so, the RiRs will undertake a brief literature review and aggregated case studies of PHM programmes across England, map the number of people and patients engaged in PHM projects/innovations across Devon, their outcomes, experiences and economic benefits which, in turn, will inform a summative evaluation of the programme roll-out. The RiRs will then expand their focus to work with other Primary Care Networks, Local Care Partnerships(LCPs) and system partners, sharing good practice, supporting evaluations and linking projects.
