Research summary
The PERFORM main trial is investigating whether the PERFORM exercise-based rehabilitation programme for patients with 2 or more long-term conditions is cost effective and works in large scale practice. Many people in the UK have multiple long-term conditions and there currently isn’t a rehabilitation programme that suits their different needs. We are looking for patients who have 2 of more long-term conditions from a long list of 44 conditions that improve with exercise and self-care. This study will be conducted at 20 sites across the UK; some of these sites will be hospitals and some will be community-based rehabilitation and wellness centres. The PERFORM rehabilitation programme lasts for 8 weeks, with 2-hour long sessions twice a week. The first hour is the exercise session and the second hour is a series of 16 health and wellbeing and self-care presentations and discussions.
There is a randomised controlled trial part of the study which will compare eligible patients on the rehab programme to those that are just getting their normal usual care, and the groups of patients are decided at random by a computer.
If a patient would normally be eligible for standard cardiac or pulmonary rehab, then they will be asked if they would like to take part in the ‘prospective cohort’ part of the study. This means that they will automatically get to take part in the PERFORM rehabilitation programme instead of the CR/PR. Some patients will also be asked if they would like to join a Facebook group with other people taking part in the trial for peer support. The trial will last for 2 and a half years. Each patient recruited will have a baseline visit at the start and follow-up visits at 3 months and then at 12-months later.REC name
