    Research Summary:
    Shared decision-making is the standard of care to reach a joint decision about care and should be practised by everyone who delivers healthcare services. Consultations for young people is complex since it involves the parent and the young person as well. Treating young people with compromised first permanent molars is complex and requires practising shared decision-making.

    We would like to analyse clinical consultations between young people, parents, and dentists to examine the process of shared decision-making in first permanent molars consultations. We will also compare the perceptions of parents and dentists to an observer in first permanent molars consultations.

    Participants will be recruited from Leeds Dental Institute. The LDI runs as a partnership between the University of Leeds and Leeds Teaching Hospitals NHS Trust. It is an NHS Teaching Hospital providing specialist treatment. Observations will be undertaken in consultation clinics in the Paediatric Dentistry and Orthodontic Departments and on the Joint Orthodontic-Paediatric clinic.

    The study will include young people aged 7-11 years with at least one compromised first permanent molar requiring a decision; attending with a parent or a legal guardian who can provide consent. Dentist involved in the consultation include paediatric dentists, orthodontists and specialty trainees or postgraduates in either speciality.

    The consultations will be audio-recorded so that all the research team members will reach a consensus on the scoring. The research team (Soumiah Alkandari, Richard Balmer, Sophy Barber) will score the consultation based on a measure we developed SDM measure (observer) V1. The recordings will not be transcribed and will be kept for 3 months to allow analysis and then deleted.

    Summary of Results:
    Treating compromised first permanent molars (cFPM) is complex and requires consideration of multiple factors to identify the different treatment options for each individual. Shared decision-making (SDM) is important to ensure dentists and patients work together to deliberate these options and reach a decision about which treatment right for the individual at the time. Our main research question was whether paediatric dentists and orthodontic dental practitioners consider all elements of SDM in consultations regarding cFPMs. Also, whether families of children with cFPMs think that SDM is practiced in child consultations. We observed and scored dental consultations using a bespoke SDM measure. The consultations were scored poorly by observers in key elements of SDM. Paediatric dentists were scored higher than orthodontists in explaining the clinical issue, treatment options, risks and benefits. Consultations showed similar trends in SDM ratings. Families rated the consultations the highest, while dentists fell in between observers and families in scoring. In conclusion, key elements of SDM were missing in consultations about cFPM with evident difference in perceptions of families, dentists, and observers in SDM. This study provides the areas where improvement could be made in SDM consultations by improving education, clinical experience, and future research.
