Research summary
The study will investigate the experience of people who have been referred for additional support to the pilot Integrated Neighbourhood Team and also the experience of their carers.
The Integrated Neighbourhood Team is a multidisciplinary team designed to ensure that the care of those aged over 75 is better co-ordinated between services in health and social care. The project is being piloted in two GP practices in Coventry - The Jubilee Health Centre and the Forum Crescent Practice.
We will conduct interviews with patients and their carers to understand from their experience what has worked and what can be improved from this pilot in Coventry, in order to inform the development of this service in the future.
