    The target conditions for this study are common mental health problems and musculoskeletal conditions.

    These health conditions account for approximately two thirds of employment sickness absence certified by a GP Fit note. It is increasingly recognised that return to work can be an important part of rehabilitation and recovery and that general practice staff have a role to play in recognising when someone is at risk of falling out of work. Early dialogue about health and work and the resulting sickness certification could be undertaken by healthcare professionals other than GPs including occupational therapists.

    This study will investigate whether it is feasible to deliver early, individualised return to work advice by occupational therapists in primary care for patients in employment who have mental health problems and/or a musculoskeletal condition.

    We aim to recruit a total of 50 patients from two GP practices, one in England and one in Wales, who have been referred by their GP or practice nurse, or self-referred to an occupational therapy vocational clinic at the practice (OTVoc). Patients will be asked to report on their health and work ability via questionnaires at baseline, and after three months.

    Semi-structured individual telephone interviews will be conducted with 20 of the patient participants, and 10 stakeholders (GPs, Nurse Practitioners, GP Front Desk staff, occupational therapists, employers) following the intervention, to ascertain their views and experiences of the clinic and the study.
