    Research Summary:
    More people are living longer than ever before, often with several long-term health conditions. This is creating bigger levels of demand for coordinated care. To address these pressures national policy is encouraging change of health and social care services not only to assure affordability but also to improve quality. However, we do not yet know how to do this.
    As part of wider research in the region and nationally, this project will carry out a case study of how the changes being put into place in one locality within South Devon and Torbay are impacting on patient care.

    This 'new model of care' places a strong emphasis on prevention and self-care, focusing on patients’ strengths and delivering care closer to home. The research will focus on two key services that operate as part of re-designed multi-professional 'Health & Wellbeing' teams in each locality: i) 'Enhanced Intermediate Care' that seeks to avoid hospital admissions and support a more prompt, safe return from hospital to home, and ii) 'Well-being Coordination' service that uses voluntary sector employees to work with patients on their goals in order to improve their health and well-being and reduce demand on services.

    The research will use a mix of approaches to understand how to improve services. This involves analysing existing service use data, service evaluations and organisational documents together with data from patient and stakeholder interviews and focus groups and observational work. Unlike traditional research, this study will take a collaborative, action-orientated approach. Researchers holding honorary NHS contracts will work closely with key stakeholders and service users and their caregivers on the ground to co-produce findings that not only help the new services improve but also help develop theory as to how and why the new care model might be benefiting care.

    Summary of Results:
    The Researchers-in-Residence programme is a partnership between the University of Plymouth and the Integrated Care System of Devon.
