    The aim of this project is to use a prediction algorithm that utilises historical patient data to rank all discharge patients according to their risk for readmission. A discharge co-ordinator is thereby assisted by a contact management tool. This enables the discharge co-ordinator to work their way down the risk ranked list, making contacts with the patients and redirecting their care if deemed necessary. The fact that the tool allows all patients to be ranked into high, medium or low risk of readmission after hospital discharge enables a prioritisation of the post discharge calls to be made. This is thus a work list of patients generated for the discharge co-ordinator who then works down the list and makes contact with the patients to make sure suitable discharge process has indeed been performed, and if there are concerns about a deteriorating patient, ensure right early intervention in one of the clinics is then made available for the patient to be reviewed early and any intervention deemed necessary medically be undertaken early, thereby avoiding a readmission.
