Research summary
In clinical practice, pelvic magnetic resonance imaging (MRI) has an established role for the evaluation of patients with perianal fistula and it is used to make clinical decisions and assess changes in disease state. Few scoring systems have been developed for MRI assessment of perianal Crohn's disease (PCD). MRI is an invaluable instrument to evaluate patients with PCD and can guide medical and surgical treatment options. The existing MRI scoring systems are not sufficiently sensitive to change and they don’t have close correlation with clinical findings, as CDAI and PDAI. We will assess a new MRI scoring system in a group of patients with perianal Crohn’s disease, treated with medical therapy and surgical therapy when necessary, to validate this score in this specific disease and to evaluate if its correlation with clinical findings is closer than previous MRI scoring systems. By collecting and assessing the data we hold at St. Mark’s about fistulising Crohn’s, we can further validate the new scoring system – enabling us to assess the link between the scores and perianal disease activity index (PDAI). Potentially for future patients, this validation will drive medical therapies and assist in making crucial clinical decisions in the patient’s care and improve patient outcomes.
