  [COVID-19 amendment 22/04/2020] The incorporation of the Consultee Declaration Form as an option within the previously approved short COVID-19 NBR joint ICF-PIS for adults v4.0, 03/04/2020 COVID-19. [COVID-19 amendment 28/04/2020] With this amendment we seek to shorten and simplify the NIHR BioResource COVID-19 research cohort PIS and ICF, which we have combined in a joint document, based on the feedback from patients and clinical staff on how to improve our existing paperwork.
