    The transition from child into adult health services is a crucial time in the health of young people with a long-term health condition, yet processes are disjointed and often fall short of what is required for effective transition. Following the successful development of an exemplar Model of Improvement for Transition at Leeds Teaching Hospital Trust, a National Transition Nursing Network is being implemented across England. This comprises of a national Lead Nurse, who is an expert in the implementation of transition pathways, and four Regional Nurse Advisors (RNAs) placed strategically across the North, the Midlands, London, and the South of England.     We are seeking ethical approval for a formal evaluation of this quality improvement model for transition, conducted by a research team at The University of Surrey, also funded by the Burdett Trust for Nursing. This research aims to evaluate the processes and outcomes of implementation of the model, documenting new learning to ensure maximum reach of the model within a network of transition care. Evaluation will be in the form of a multi-centre mixed methods study design with qualitative (case studies and interviews), and quantitative descriptive (surveys). These data will be collected simultaneously over three phases, spanning approximately two years. Participants will include young people between the ages of 12 and 25, their parents, key professionals involved in the young person’s transition journey, the national Lead Nurse and the RNAs. It is expected that this national implementation will not only raise the profile of transition but will also improve the experience for young people and their families, and improve knowledge and understanding of their needs. It is anticipated that this will, in turn, improve outcomes for young people transitioning to adult services.
