    Does the MY COMRADE PLUS MultimorbiditY COllaborative Medication Review And DEcision Making (MY COMPRADE) intervention help general practitioners (GPs) and Practice-Base Pharmacists (PBPs) to actively review the medications of people with multiple chronic conditions, known as multimorbidity?

    Multimorbidity is the term used when a person has more than one long-term medical condition. In general practice, patients are more likely to have multimorbidity than a single condition. For patients with multimorbidity, there are many challenges, one of which is the management of several medications.
    Internationally, people are looking for new approaches to prescribing for patients with multimorbidity. The aim of this study is to test one such new approach north and south of the border in Ireland.

    We have conducted three studies to explore the difficulties GPs have when prescribing for people with multimorbidity. Our work has shown that GPs respond to these difficulties by discussing challenging cases with each other.

    Using the findings of our earlier research, we developed a new approach, which we call the MY COMRADE approach. MY COMRADE requires two GPs to set aside time to review all the medications a patient with multimorbidity is on, using a special checklist to help them with the review. In Northern Ireland, reviews will be conducted by pairs including a GP and Practice-Based Pharmacist (PBP).

    Before testing MY COMRADE in a large scale trial, this 2.5 year pilot study is required to see if it is possible to use the MY COMRADE approach in practices and to see if GPs and the PBPs are willing to use this approach.
