Research summary
We propose a pilot study to conduct serial Magnetic Resonance Imaging (MRI) and histological sampling on pancreata, which we deem to be untransplantable, prior to disposal. We aim to be able to identify whether there are markers of deterioration visible on MRI, which correlate to histological samples, in relation to increasing cold ischaemic times (the time from retrieval of an organ to implantation). We we also be assessing the preservation fluid for identifiable markers of pancreatic deterioration.
Once we have data about how structural pancreata change with increasing cold ischaemic times we aim to conduct pre-operative MR imaging of pancreata prior to implantation and correlate these findings with post-transplant outcomes.
We aim to process 10 pancreata. Initially we would like to conduct scans every 2 hours, up to a maximum of 3 scans per organ (and a biopsy at each time point). Once we have analysed this data from the first 3 organs we would expect that fewer scans and biopsies per organ will be required for analysis.
The organs will be transported to the Welcome Trust MRI facility in a specific carrier box inside which the organs will be in a solution in 3 sealed, sterile plastic bags. The sterile carriers will be scanned so no handling of the organ is necessary. Following the final scan the organs will be transported back to Manchester Royal Infirmary in accordance with Human Tissue Authority (HTA) license and National Health Service, Blood and Transplant regulations.
