    The study is investigating a subset of participants undergoing knee distraction to treat symptoms of osteoarthritis as part of a larger NIHR HTA funded randomised controlled trial (KARDS). The KARDS trial is investigating knee distraction in patients under 65 who need a knee replacement to see whether knee joint distraction can improve symptoms and delay need for a joint replacement. Participants will have an external frame attached to their leg for 6 weeks. Participants will be randomised to knee distraction or the standard treatment of knee replacement and will have imaging of radiograph only.

    The aims of this additional study are to assess the quality and quantity of regenerated articular cartilage in patients who have undergone joint distraction for OA and to examine the biological mechanism associated with improved cartilage regeneration following joint distraction.
    For this additional study on a subset of KARDS patients MRI will be taken pre operatively in all patients, and at 10-14 weeks post surgery and 10-14 months post surgery in the knee joint distraction patients.
    Joint fluid samples will also be taken from the knee. In participants undergoing knee replacement this will be at the time of the knee replacement under anaesthetic. In participants undergoing knee joint distraction this will be at the time of application and removal of the external fixator under anaesthetic. These two samples of joint fluid will be analysed for markers of cartilage recovery. Joint fluid samples and articular cartilage will also be taken at the time of knee replacement.

    100 patients from a single centre will be included.
