    ‘Polypharmacy’ is a term generally used to refer to the use of many different medicines by one person. If someone is taking many different medicines this increases the risk of medication errors, side-effects and drug interactions, and makes it more difficult to take the medicine as directed. Polypharmacy is particularly common in older people with many different illnesses including older people in care homes. In line with recent National Institute for Health Care and Excellence (NICE) guidelines on medication management in care homes for the older people, the pharmacists within Wolverhampton CCG regularly review medication of people in care homes.

    This study aims to explore the views of pharmacists, general practitioners, residents/relatives of residents and care home staff on the impact of these medication reviews on patient care. Wolverhampton CCG will identify possible care homes, GPs and pharmacists for the study. Care homes who agree to be involved will identify possible care staff, residents and relatives. Brief information on the study will be sent to these groups.

    GPs, pharmacists, care staff, relatives and residents who agree to be involved will be interviewed. The interviews will last about 30 to 60 minutes and cover their experiences and views of the reviews and the effect of the reviews on patient care. Data will be analysed to identify key themes; particularly views on the medication reviews. The findings from the study will inform how the service is shaped in the future.
