Research summary
In 2007 the National Institute for Clinical Health and Care excellence (NICE) and the National Patient Safety Agency (NPSA) produced a guideline on Medicines Reconciliation. However, children under 16 were excluded from this guideline. The guideline recommended that all patients over 16 years of age should have Med Rec undertaken within 24 hours of admission to hospital. At the Alder Hey Children’s Hospital NHS Foundation Trust and the Countess of Chester NHS Foundation Trust this policy has been adopted for patients of all ages. Trusts’ policy is to undertake Medicines Reconciliation for all new patients seen by clinical pharmacists or Medicines Management technicians. The Trusts’ Standard Operating Procedure (SOP) for medicines reconciliation and drug history taking states, where possible, that more than one source of information should be used to complete a medication history and that patient/parent/carer consultations are an essential part of this process. Other sources which may be used to obtain an accurate drug history include: patient’s own drugs (PODs), Discharge summary letters (Medisec), Summary Care Record (SCR), Cheshire Care Record, information from GP surgery, repeat prescriptions (of value if issued within the last 6 months), previous ‘To Take Home” (TTOs) prescriptions, electronic prescribing system or medical case notes. We would like to ascertain if GP records should be requested for every single paediatric patient as it is the case and the recommendation for adult patients ('Gold Standard' for Medicines Reconciliation')
