    Heart failure, where the heart is unable to pump enough blood around the body to meet requirements, can be a common problem especially in older people with other conditions. It is estimated a third of people diagnosed die within the first year. However, people are often not referred to specialist palliative care (specialists in end of life care)until the end of their life, if at all. Joint working between heart and end of life specialists has been found to have a positive impact on patients and informal carers. We are proposing early introduction to palliative care expertise will help a person with heart failure to live well with heart failure and be aware of how to control symptoms and how to seek help and support. Furthermore, informal carers have been found to influence how the person lives with heart failure and so we are offering for them to join in with the programme. We want to offer an 8 week programme for people and their informal carers to attend at a day hospice to see if quality of life, symptom burden and carer burden can be improved.
