Research summary
The way professionals work together is proven to improve health outcomes for people with one health condition. What remains unknown is whether a specific way of professionals working together in teams which respect one another called Interprofessional Collaborative Practice can improve outcomes for people living with multiple long-term conditions.
By exploring research already carried out in a scoping review I will be able to identify gaps in this area of work. I will gather information by preparing questions to discuss with people in my study. I will also carry out a questionnaire of health and social care professionals working with people with long term conditions. I will analyse this information at the same time.
I expect to describe and understand what enables professionals working together with patients and their families to achieve the best outcomes for people living with multiple long-term conditions. I expect that I will also find out what stops this way of working from happening. Both findings could help me to develop better ways of working together which might be sustainable and achievable for the workforce and provide the right way to improving outcomes for people living with multiple long-term conditions and their carers.
There are four main groups of participants, people who lead organisations which provide health and social care, health and social care professionals who provide direct care to people living with multiple long term conditions, people who live with multiple long term conditions, and their care givers or family members. These participants must be over the age of 16. These groups will be either providing or receiving health and social care in NHS Fife, or in Fife Health and Social Care Partnership. They may be receiving care in a variety of settings.
