    The Three Dimension for Long Term Conditions study aims to provide integrated mental and physical health care for individuals with long-term conditions (LTCs). This will be through scaling up the 3 Dimensions of Care for Diabetes (3DFD) service to other LTCs. We will do this by integrating with existing secondary and community care services in Lambeth and Southwark, generating a care-pathway for complex patients. In particular, focusing on patients diagnosed with heart failure, COPD and hypertension. Research evidence has shown that integrated mental and physical healthcare is associated with better outcomes, but little clinical evidence in ‘real life’. 3DFD has demonstrated that this approach is successful in South London at improving physical healthcare outcomes. In addition, it has demonstrated improvements in psychosocial measures in a deprived inner-city population of people with poorly-controlled diabetes. The overall objective of 3DLC is to a) assess if scaling-up a disease-specific model to a multi-condition model leads to improvements in health and economic outcomes, and quantifying the effectiveness, efficiency and acceptability of the 3DLC b) identify the processes effective in delivering the scaled-up interventions.
