    Integrated care is an approach to organising services that focuses on coordinating care around the needs of patients. Although current policy supports integrated care, there is little empirical evidence to such development. This study will develop a rich understanding of integrated care in practice as it develops in three London boroughs for older, frail people and those with complex long term conditions. A mixed-methods action research approach will gather data for an in-depth case study that will illuminate the challenges of achieving integrated care and develop and implement a methodology which will achieve iterative quality improvement of integrated care efforts.
