    This project is part of a larger programme evaluation of the Supporting and Maintaining Independence Programme (SMIP) in Doncaster. The evaluation team from Sheffield Hallam University is working in partnership with Doncaster Metropolitan Borough Council (DMBC). This proposal brings together two of the main workstreams of the wider evaluation and forms those elements that require NHS Research Ethical approval. The main aim is to undertake an in-depth qualitative evaluation of two investment areas of the Supporting and Maintaining Independence Programme (SMIP): the Hospital Discharge Pathway (HDP) and associated discharge pathways. The focus will be on client and staff experience, impacts and outcomes. The research will have two main elements. The first element will be a qualitative longitudinal cohort study which will follow a group of clients going through the HDP, discharge destinations and engaged Community schemes. This will include interviews with the client and up to two key stakeholders in their care e.g. a family member or care giver. This element will capture their experience of the services and how the services contribute to client need and outcomes e.g. quality of life. Recruitment will take place at two points, summer and winter. The idea is to follow clients’ journeys in order to track client experiences and expectations of services and the outcomes for clients. The second element of this research will take a collective case study approach to explore frontline and managerial experiences, operational challenges and opportunities in meeting performance indicators and outcomes, and user experience of particular services. These cases will be identified and recruited in consultation with the SMIP management team and service staff. It is anticipated that the three cases will be Home from Hospital, the Wellbeing Service and the Social Prescribing Project.
