    Supported self-management, which includes providing Personalised Asthma Action Plans, helps people adjust their treatment in response to changes in symptoms.  Studies show that this improves day-to-day control and reduces the risk of asthma attacks and time off work/school. However, for many reasons, supported self-management is not widely implemented: fewer than a quarter of people replying to a recent Asthma UK web-survey owned an action plan.
    A review of existing research on self-management of asthma determined that successful implementation of self-management required attention to patient resources, professional motivation and training, and organisational prioritisation and support. In our preliminary IMP2ART programme development grant (PDG) we explored key components of this ‘whole systems’ approach to implementing supported self-management in general practices across the United Kingdom.
    This study is the developmental phase of the programme of work in which we will develop and refine patient/professional/organisational components of a whole systems implementation strategy. Components of the strategy (such as a new action plan, new review template, new review invitation letter, audit reports) will be piloted by participating practices and feedback (interviews/e-mail) sought from professionals and/or patients as appropriate.
    The findings of this preliminary work will inform an internal pilot of the implementation strategy before it is tested in a UK multicentre cluster randomised clinical trial.
