Research summary
Hospital pharmacist led medicines use reviews are a proposed mechanism for reducing errors at the interface between hospital and community care. This investigation will specifically test how a specialist cardiology pharmacist influences the quality of care delivered to patients suffering from ST elevated myocardial infarction at discharge from an acute coronary care unit, when utilising a novel electronic report called a ‘Specialist Pharmacist Discharge Review’ (SPDR). The report will include details of changed medication and information on how newly prescribed medication should be managed seamlessly in the community. In addition to describing the development of the SPDR this review will examine the role the specialist pharmacist plays in influencing the care delivered to patients when discharged to GP care.
