    The rapid rise in the prevalence of type 2 Diabetes globally, is an urgent health and economic concern. The development of End-stage renal disease (ESRD) due to diabetes confers a huge mortality and morbidity burden on patients, with life expectancy less than that of many common cancers (breast colon, ovarian or prostate). Up to 50% of patients starting dialysis in East London have diabetes as the cause of their end-stage renal disease. Taken together there is a huge need to better understand the progression of diabetes from chronic kidney disease to ESRD to enable better prevention and management strategies for the decades ahead.
    East and North Central London includes some of the most ethnically diverse and socially deprived areas of Europe. These areas suffer from some of the highest rates of DM and associated complications. This is a cohort study whose goal is to:
    • Accurately define those with DN who are at risk of progression, by histological diagnosis.
    • To collect tissue, biofluids and imaging data early in the disease to identify modifiable biological mechanisms, rather than common end-stage disease pathways.
    • To characterise the evolution of the disease and gain an understanding of pathogenic factors, avoiding problems of reverse causation.
