Research summary
Background: Up to 50% of fistula (a bridge between the arteries and veins) formed for renal replacement therapy fail within the first year. Currently very little is understood about the early maturation process of the blood vessel after the operation. Objectives: To investigate the early stages of blood vessel remodelling after fistula formation for renal replacement therapy. Design: We will use MRI and ultrasound to monitor the size of vessels and the blood flow within the fistula in the early pre and post-operative period. A subgroup (n=5) will have pressures measured within the graft using a catheter (hollow tube) at operation and during routine clinical fluoroscopic assessment of the fistula. In total 7 individuals will be assessed with MRI and 40 healthy volunteers with ultrasound. The HV US data will be compared with US data obtained retrospectively from 60 end stage renal patients before and after their fistula formation. Data will also be collated from Ninewells databases in a bid to identify markers of fistula failure. Expected outcomes: The majority of vascular remodelling and changes in the blood flow will occur early in the post-operative period. Anticipated healthcare outcomes: Increased knowledge about fistula maturation, which will improve understanding about factors that may help predict failure of maturation.
