    GREAT-iP is a service improvement project with a research element that builds on a recently completed trial (Goal-oriented cognitive rehabilitation in early-stage dementia: multi-centre single-blind randomised controlled trial (GREAT). Cognitive rehabilitation (CR) is a person-centred intervention that helps people with mild to moderate dementia maintain independence through managing everyday activities better. Several research studies, including the GREAT trial, have found positive results. This individualised therapy is conducted in people’s own homes over several sessions with a practitioner who works with the person and the care partner to identify realistic and personally-meaningful goals, plan ways of addressing these, and support goal attainment.

    In this project, we will begin the process of enabling NHS memory services and social care providers such as home care or community reablement services to offer CR, by working with at least 15 NHS, local government, and private and non-profit care provider organisations. We will work with stakeholders to adapt the intervention to real-life practice and produce informative and engaging training resources for staff and guidance for decision makers. We will then support our partner organisations to introduce the intervention into practice and sustain the changes made. Full involvement of people with dementia and care partners will ensure the intervention is implemented in a way that meets their needs and preferences. We will evaluate the extent to which each organisation meets its goals and identify what the benefits are for people with dementia and care partners and for the organisations. At the end of the project, we expect that GREAT CR will be effectively implemented in at least 15 organisations, and a tried and tested strategy will be available for wider implementation throughout the UK.
